ID Cards

Accurate and complete information on medical identification cards is the key to successful claims submission and repricing. Before producing or issuing new cards, please send a proof to Payer Relations for approval.

Midlands Choice requires:

• Front:
  • Payer/administrator name
  • Employer or group name and number
  • Midlands Choice logo


• Front or Back:
  • Claims mailing address
  • Claim administrator's phone number, mailing address and Midlands Choice EDI Payer ID 47080
  • For national networks such as MultiPlan/PHCS, include the network's logo with appropriate wording, such as Out of Area, Wrap Network, Secondary Network or National Network

EOBs

Midlands Choice must be identified on EOBs.
