In order to be compliant with the No Surprises Act and not be excluded from the Midlands Choice online directory, providers must attest to the accuracy of their provider information every 90 days by following the steps outlined below.
To review the demographic information for providers listed under a Tax Identification Number (TIN) and alert Midlands Choice to any updates:
- Go to www.midlandschoice.com/attest
- Enter the TIN and click ‘Search’
- A box will appear asking to attest to the accuracy of the provider information and to select an answer by clicking on the dropdown arrow.
- If ‘Yes, I agree to attest to the accuracy of our provider information’ is chosen:
- The reviewer will be asked to enter their name and email address and click ‘Submit’.
- The Roster Report for the TIN entered will display. The report may be exported to Excel.
- The report must be reviewed to verify that the information is accurate.
- Report any changes, additions, or terminations to database@midlandschoice.com.
- If the information on the report is correct, no further action is required.
- If ‘No, I will not be attesting to our provider data accuracy at this time’ is chosen:
- The box will continue to appear each time the TIN is entered in the Roster Report until the provider agrees to attest to the accuracy of the provider information.
- If verification of attesting for accuracy is not received, the providers under the TIN will be excluded from the Midlands Choice online provider directory.
