The employer groups Shenandoah Medical Center and KanEquip Inc have moved coverage to Unified Health Plan LLC, a new administrator with Midlands Choice. The groups were previously with ValueHealth Benefit Administrators, also knows as Benefit Management LLC. Claims for members of these groups should be submitted to Midlands Choice using EDI ID 47080. For benefits, eligibility and claim status, call Unified Health Plan at 800-259-8330. For precertification, call Partners Health Alliance at 844-742-9675.
