Assured Benefit Administrators is a new payer with Midlands Choice. Claims for members of Assured Benefit Administrators should be submitted to Midlands Choice, as is listed on the member identification card.
Effective January 1, 2023, two groups will be joining Assured Benefit Administrators that will access the Midlands Choice network:
- Des Moines Orthopaedic Surgeons, P.C. will begin coverage through Assured Benefit Administrators effective January 1, 2023. The group numbers are ABA8888 and ABA8887. For eligibility, benefits, medical necessity reviews and claims status, contact Assured Benefit Administrators at 800-222-8734. For preauthorization, call HiCard at 800-850-0281.
- Cedars Youth Services will begin coverage through Assured Benefit Administrators effective January 1, 2023. The group numbers are ABA8884 and ABA8885. For eligibility, benefits, medical necessity reviews and claims status, contact Assured Benefit Administrators at 800-222-8734. For preauthorization, call HiCard at 800-850-0281.
