CoOportunity Health contracts to use the Midlands Choice provider network. For questions about credentialing, contact Midlands Choice at 800-605-8259 or at cs@midlandschoice.com. Continue to notify Midlands Choice of demographic changes 30 days before they are effective.

CoOportunity Health contracts with HealthPartners Administrators, Inc. to provide claims processing, medical management and certain other administrative services on their behalf. 

Please work with your vendor to verify that a connection is established for electronic data interchange (EDI) needs with one of the listed clearinghouses as claims and other transactions will be submitted to HealthPartners, not Midlands Choice.
