To avoid a denial, please file claims using each patient’s unique CoOportunity Health identification (ID) number. Each family member, regardless of age, receives an ID card and number. These numbers are not necessarily sequential for family members. To locate the appropriate ID number, enter the patient’s name and birth date using CoOportunity Health’s secure Eligibility application.
