Effective January 1, 2014, Midlands Choice established a new payer relationship with Bernie Lowe & Associates, Inc. to use the Midlands Choice network in the administration of benefits for municipalities located in Iowa covered by the Iowa 411 law. The 411 law specifies that “cities shall provide hospital, nursing and medical attention for the members of the police and fire departments of the cities, when injured while in the performance of their duties as members of such department.” Claims that are determined to fall under this law will be administered through Bernie Lowe & Associates.

Please be aware that patients may present their medical ID cards at the time of their initial visit and at a later time be directed to resubmit their 411 program card due to eligibility determination. It is important to follow the instructions on the back of the member’s 411 program card for claim submittal information. Claims that are submitted to Midlands Choice will be rejected to the provider to resubmit to the correct address.
Claims incurred through this plan will be considered at 100% of the Midlands Choice contractual allowed amount with no member responsibility and are funded by the municipality. Benefits for these municipalities’ 411 plans were previously administered through First Administrators, using their network.
