In late 2013, Midlands Choice was notified that the employer group Revstone Castings Fairfield (formerly Dexter Foundry) located in Fairfield, IA was to terminate their relationship with Midlands Choice as of December 31, 2013.  The administrator for Revstone Castings Fairfield will continue to process claims for 2013 dates of service until March 31, 2014.

After March 31 there will be no staff available at the former administrator to process payments or answer questions regarding previously adjudicated claims.  Therefore, providers are encouraged to contact the payer directly at (866) 926-8409 to resolve any outstanding issues.  Any 2013 claims not yet submitted for members of this group should be submitted to Midlands Choice immediately for repricing and submission to the group for payment.
