Starting on January 6, 2014, the 1500 version 02-12 claim form became available for use by providers submitting professional claims on paper. Providers have the option of using the old version of the form (CMS 1500 v. 08-05) or the new version (1500 v. 02-12) until the March 31, 2014 compliance date, after which time only the version 02-12 form must be used. Paper claims for professional services that Midlands Choice receives submitted on the CMS 1500 (08-05) form after March 31, 2014 will be sent back to the provider requiring that the claim be resubmitted on the 1500 (02-12) form.
