Centivo is a new payer with Midlands Choice. Effective January 1, 2022, there are two new groups joining Centivo that will access the Midlands Choice network. Claims for Centivo should be submitted directly to Centivo as listed on the ID card.

Nebraska Furniture Mart (Des Moines, Iowa employees only) will begin coverage with Centivo effective January 1, 2022. The group number is NFMEX. Claims should be submitted to PO Box 211681, Eagan, MN  55121 or electronically using payer ID 45564. For precertification, to confirm eligibility, verify benefits, or check claim status, contact Centivo at 877-293-5521. 

 

Charon Shared Services is currently with Cigna and will be moving to Centivo effective January 1, 2022. The group number is CHRON. The network features One Health Nebraska providers as Tier 1 and Midlands Choice providers as Tier 2. Claims should be submitted to PO Box 211681, Eagan, MN  55121 or electronically using payer ID 45564. For precertification, to confirm eligibility, verify benefits, or check claim status, contact Centivo at 844-993-3165.
