Beginning January 1, 2021, Bright Health will begin utilizing the Midlands Choice network for their commercial business in the state of Nebraska. 

Claims for Bright Health’s Commercial Plans should be submitted directly to Midlands Choice. The group number for these plans, which is displayed on the member identification cards,  will begin with BHGNE. The Midlands Choice logo is also displayed on the ID card, as well as the claims mailing address of PO Box 5809, Troy, MI 48007-5809, and the payer ID 47080.

To confirm eligibility, verify benefits or check the status of a claim, call Bright Health at 855-521-9364.  The pre-authorization fax number is 833-903-1067. To obtain authorization for hospital admissions, go to availity.com.
