Medica is offering a new individual and family (IFB) health plan product in addition to the three current IFB products in Nebraska and Iowa that will be in effect January 1, 2019.
Elevate by Medica is a care system product being offered in the individual and family business (IFB) market in select counties in Nebraska and Iowa. The network features Methodist Health System and Nebraska Medicine doctors plus others in southwestern Iowa and southeastern to central Nebraska. No referral is needed to see in-network specialists. This product is available to individuals in Mills and Pottawattamie counties in Iowa and Douglas and Sarpy counties in Nebraska. To verify which providers are included in this product's network, see the provider directory on Medica.com. Claims for this product should be submitted directly to Medica using payer ID 12422.
