Midlands Choice has changed the process for sending termination notifications. Only one letter will be sent that is labeled “Termination”. The termination letter will be mailed approximately two months prior to the termination date. If a recredentialing application is not received in time for the credentialing department to process the application, the provider or facility will be terminated on the date indicated in the letter.
