All demographic changes for group practices or specific providers must be communicated to Midlands Choice or your contracting entity. The receipt of the updated information is imperative so that claims will reprice accurately.
The following are examples of issues that should be communicated. It is also important to include the effective date of the change.
- Address changes
- Additional service locations

Providers leaving your practice
- New providers added to your practice
- Any change to the group practice name
- Tax Identification Number (TIN) changes
- Any changes to the billing and/or remit to addresses
