Pursuant to the Liquidation Order of the Iowa District Court, all claims for healthcare services provided to CoOportunity Health insureds must be submitted by December 15, 2015.  Late filed claims cannot be paid unless there are sufficient assets in the CoOportunity Health liquidation estate to first pay other creditors in priority classes higher than the late filed claims classification.
  
All CoOportunity Health policies were terminated by August 31, 2015.  Claims for services to CoOportunity Health insureds which are properly submitted via the established procedures by December 15, 2015 are sufficient to establish a claim in the CoOportunity Health liquidation proceedings.
  
Please do not submit duplicates of claims previously submitted.
