Cigna has updated their precertification list to reflect HCPCS codes released by The Centers for Medicare & Medicaid Services (CMS) on October 1, 2012.  In addition, the following precertification list updates have been made from what was previously published in Cigna’s July, 2012 newsletter:

• Cigna’s July publication indicated that code C9276 would require precertification as of August 6, 2012.  That code was replaced by J9043.
• Cigna’s July publication indicated that code C9399 would be added to precertification as of August 6, 2012.  At that time, this code was added to represent Eylea.  CMS has since released code Q2046 (Injection, Aflibertcept, 1 mg).
